There are two levels of support — see which one fits you.
Every New York Medicaid member can get free screening and referrals. Some members also qualify for hands-on services, based on their plan and health needs.

Direct services
For Medicaid Managed Care members who also have a qualifying health or life circumstance — for example a health condition, pregnancy, recent hospital stays, or care coordination through a Health Home.

Does this cost anything?
No. Screening, navigation, and qualifying services are all free to Medicaid members.
Will this affect SNAP or WIC?
No. Using the Social Care Network doesn't change your eligibility for SNAP, WIC, or other benefits.
Do I need a referral?
